test(cqrs): 补齐 fallback 元数据回归

- 新增 mixed fallback 禁用多实例 attribute 时的字符串回退覆盖

- 补充 runtime AttributeUsage 变体测试辅助方法

- 更新 CQRS 恢复文档与本轮验证记录
